Services range from the high-tech, high-intensity, and highly specialized to those that are more consultative and educational, 24 hours a day, 7 days a week.

Sunshine Home Care professionals speak multiple languages to ensure the patient’s comfort and full understanding of their health care protocol.
All patients are accepted for care regardless of race, color, sex, ethnicity, religion, age or disability.
Protocol to Reduce Readmissions
Sunshine’s policy is to admit patients to home care within 24 hours of discharge. Whenever possible, a Sunshine health professional will visit the high-risk patient in the hospital before discharge.
On the first home visit, the Sunshine home health nurse conducts an assessment and plans follow-up care as needed
Visits occur more frequently at the beginning of care to ensure at-home success.
Sunshine Home Care professionals provide patient education in the following areas:
Medication Reconciliation— To make sure prescriptions have been filled, to assist the patient with medication education, and to prevent medication errors in the home.
Patient Personal Health Record—To provide a personal health record for the patient, to assist the patient in documenting all health information in the record and to educate the patient on the importance of providing this information to all health care providers .
Follow-up Appointment Management— To assist the patient with management of follow-up appointments, from making the appointments to arranging transportation.
Disease Education to Prevent Readmission— To educate the patient on the disease process, the red flags that indicate worsening of his/her condition, steps the patient needs to take in obtaining early help in managing the symptoms, and how to prevent unnecessary readmission to the hospital.
If at any time the patient requires medical treatment, the Sunshine home health nurse will contact the Primary Care Physician and obtain medical orders.
Ultimately, symptoms are managed at home, limiting or entirely preventing returns to the hospital. The emergency room is a last resort.
